What vitamin B12 actually does
B12 plays a role in three systems that affect how you feel every single day:

Early signs of vitamin B12 deficiency
Symptoms usually build gradually, so it helps to know what to watch for before they become severe.

Pale or yellowish skin. B12 deficiency can affect red blood cell production enough to change your skin tone.
Tingling or numbness in the hands and feet. This happens because B12 supports the health of your nerves. Left unaddressed, it can progress over time.
Difficulty concentrating or memory lapses. Often described as brain fog. People frequently attribute this to stress or lack of sleep, when the actual driver is nutritional.
Shortness of breath and dizziness. Since B12 affects oxygen-carrying red blood cells, even mild exertion can feel harder than it should.
Mouth and tongue changes. A sore, red, or unusually smooth tongue, along with mouth ulcers, is a lesser-known but common sign.

Vegetarians and vegans. B12 is naturally found almost exclusively in animal products. Anyone eating a fully plant-based diet needs a reliable supplement or fortified food source to avoid deficiency over time.
Adults over 50. Stomach acid production naturally declines with age, which reduces how efficiently B12 is absorbed from food, even when intake looks adequate on paper.
People with digestive conditions. Celiac disease, Crohn's disease, and any condition affecting the small intestine can interfere with B12 absorption regardless of diet.
Pregnant and breastfeeding women. B12 needs increase during pregnancy and lactation to support the baby's development.
Why B12 is different from other nutrients
Most nutrients your body needs can be reasonably estimated from what you eat. If you know someone's meals, you can get a fairly accurate picture of their iron, vitamin D, or protein intake. B12 doesn't work that way. Absorption depends on stomach acid, gut health, and factors that have nothing to do with the food itself, which is why diet alone isn't a reliable way to estimate someone's B12 status. The only accurate way to know your levels is a blood test ordered by a doctor.
